Monthly Parking Application Form

The undersigned agrees to the following terms and conditions:

    1. Access Card: Your access card may only be used to exit the parking facility.
    2. Monthly Fee: A monthly licensing fee, including sales tax, is charged for each vehicle parking in the facility every month. The fee may be increased only after a 30 day notice is given.
    3. Payment Terms and Due Date: The monthly fee is due by the 1st working day of the month. If payment is not received by the third of the month the access card will be deactivated. A $15.00 reactivation fee will be charged if your card was deactivated. No discount is given for partial months. Refunds are not given for days the permit was not used.

    5. Invoicing: Invoices can be sent to companies with over 5 permits. Invoices will be sent out by the 15th of the previous month and should be paid by the 1st working day of each month.
    6. Display of Parking Permit: The monthly permit must be displayed on the vehicle whenever parked in the facility.
    7. No Bailment Created: Please realize that no Bailment is created. Both CBD Management, LLC and undersigned agree that this is a licensee/licensor agreement and CBD Management, LLC is not taking your vehicle into our care, custody and control. Damage to your vehicle caused by a third party is not the responsibility of CBD Management, LLC or the garage owner. CBD Managment, LLC assumes responsibility only if CBD Management, LLC is deemed negligent.
    8. Cancellation & Return: Please return your parking card to a CBD representative if you decide to no longer park in the parking facility.
    9. Replacement Cards: Access cards or permits that are lost or damaged by the customer are subject to a $25.00 fee.
    10. Liability: Neither CBD Management, LLC or the owners of the parking garage or its employees are responsible for any vehicle or its contents by fire, theft, collision or any other cause. It is the responsibility of the vehicle owner to remove all valuables from your vehicle. In the event a vehicle owner suffers any loss or damage to property, the vehicle owner shall seek coverage from their own insurance and shall not make a claim against the parking management company or the parking lot owners.
    11. Vehicle Storage: The permit is intended for daily use and the vehicle must be removed on a daily basis. Vehicles cannot be stored in the facility for more than two days. Management reserves the right to remove or demobilize the vehicle if left in the garage for more than two days. Please contact the garage manager for any extenuating circumstances.

    The undersigned customer agrees and accepts all the terms and conditions hereof and releases CBD Management, LLC and the garage owner from any and all liability arising out of the use of the parking facility unless CBD Management, LLC is deemed negligent.
